I couldn't find this set anywhere in the database and this is the only card I have.  Anyone have a checklist?

COMC has five of the six.

I'm a bit confused as there doesn't seem to be a lot of consistency, should this be submitted as an Insert/Related set under the main 1992 Pacific or as a completely separate set under Promo?

I would put these under a promo set in the Insert/Related set for 1992 Pacific. These six cards were handed out at the Super Bowl card show in Minneapolis to preview the 1992 Pacific set. The 1992 Action Packed Prototypes were also handed out during the same Super Bowl card show and are listed in the Promo section of 1992 Action Packed.
