Distributed in 3-card packs at participating McDonald's restaurants. Packs were free with the purchase of an Extra Value Meal or 59 cents with the purchase of another menu item. Restaurants in the Boston, Chicago, Cleveland, Los Angeles and Orlando areas had packs that contained a local team card in place of the third player card or instant winner card, and those local sets are listed under Inserts/Related.

Inserted one per Boston regional pack. Card numbers have a "BT" prefix.
Upper Deck McDonald's - Chicago Bulls
Inserted one per Chicago regional pack. Card numbers have a "CH" prefix.
