That could be the case with that Frank Thomas card. I saw an IR stating that the scan was actually a reprint, not an original. I don't know if that's true or if that's what happened as I left it alone due to said cluelessness.
There are three things that can make a scan bad: washed out, poorly cropped, or a card the scanner can't handle. Any of those or any combination of those (1 and 2 tend to go together, but not always) are good reason for a scan to be replaced. Also, something like a water mark or hair present in the scan, both of which I've seen posted, also need to be replaced even if the scan is otherwise perfect.
I will gladly give a list of scans I've submitted that need replacing. (A partial list as I am sure there are some I'm not remembering) The scanner I used from 2012-15 could not handle some kinds of cards, but, not having much foresight, I didn't think I would ever get anything better so I posted them anyway. In 2015 I bought a new scanner that COULD do them without the lines. Eventually I will go back and rescan them, even if somebody else posts corrections first.
The lines present in the orange and across his face- especially in his neck- means this is a crappy scan and never should have been posted. I regret doing so now, and if I knew where the cards were, I'd redo them. I plan to eventually, when I find them.
Conversely, the scanner I bought in 2015 that CAN handle the cards the older scanner can't, cannot handle Stadium Club cards or old school brown back cardboard, among others. Luckily, I still have the one I used from 12-15 and pull it out when I need to, one of 4 working scanners I use as needed.
Here's an example of a scan that has all three things wrong with it- it's extremely washed out, it's not properly cropped, and the scanner could not do the card properly- note the lines in his face and neck.
My scanner I used from 12-15 could not handle 1993-94 Ultra either, but my new one can. I will replace this scan at some point, whenever I get my copy of this card scanned.

No lines, properly cropped, and not so washed out that you can't read the small text. The goal is to have the scan look exactly like the card does in hand.
VERY slow trading due to health problems. Not transferrable so safe to trade with, just moving is painful and can't always access the cards.
