Like Mark, I also don't sleeve my cards before putting them in pages/binders. There is an exception though. All vintage cards (pre-1980) get sleeves before they go in a page. But not all vintage cards go in pages.
As for a card in a One-Touch in a page, that seems like expensive overkill. A card placed in a penny sleeve, then toploader, then sealed in a team bag placed inside of a plastic page is plenty of protection. One-Touch cases are much more expensive and are plenty of protection on their own. I do place my One-Touch cases inside of team bags to protect the case from scratches, but they don't go in albums.
In the situation of a card in my Player Collection not being in the album with the rest (due to being cased or graded/slabbed), I also do like Mark said and put a note in the right spot telling what the card is and where to find it.
