Found this card in my collection, but I have only seen this with MEM.

I was going to ask, where are the GU materials that were certified to UD? Strange.

Maybe they ground the game used hockey sticks up into paper pulp to make the cardboard for the card.......

That's just the COA card. The MEM card that goes with it is an 8-player back-to-back (4 each side) card. And I see we don't have the COAs listed.

When this insert set is pulled, you get two cards for it. One a front and back of 8 jersey swatch pieces. The other card is what you have, shows who will be on the jersey card and the "Congratulations" part.
Here's an example of an eBay auction where they have both cards. Chicago / Detroit
