The 16 Lone Ranger Dell Comic Cover cards in the checklist were originally four rows of four cards on one perforated sheet. How do I list the perforated sheet in TCDb? Is the perforated sheet called an uncut sheet? If so, how do I add the uncut sheet? Submit an IR request?

I would put it as unopened material and specify in the comment that it's a full sheet. 

List it under 'Inserts and Related sets'.

The full sheet can only be added if that was the only means of distribution of this set. If they were available as single cards (in various products), the sheet cannot (and should not) be listed.

Only if uncut or perforated sheets were the primary way the set was released, will they have a checklist created.   Other wise they won't.   I have added uncut sheet under thier respective set under >packaging >set, this way we have atleast documentation that they existed.

The 16 Lone Ranger Dell Comic Cover cards were released as a Merita Bread Lone Ranger Safety Club Premium through the mail. The 16 cards on a perforated sheet were the primary way the set was released.
