Card not on database, unsure of the set

Hi Everyone.

I have a 2002 Trevor Hoffman Upper Deck card with the Papa John's Logo on it celebrating Trevor's 300th career save. It has the Upper Deck logo on it and it says it's card 2 of 3. I got this card a very long time ago, maybe even on 2002. An extensive Google Search yielded no information about this set of 3 cards, and I'm missing the other 2 cards from that set.

Neither set nor card are listed in the database for me to add to my collection. I'm sure it's an official licenced card, even if it's only promotional. How do we go about adding this card to the database? I can send a scanned image for review if needed.
