2004 Green Bay Packers Mike Sherman # 1

First off I know the card is not in great shape.

I also know that there are 12 cards for Mike Sherman listed for 2004 with the # 1. They all look the same as this one but the organization on the cards are different. I can't match the Navigator Planning Group up with any of the cards listed. The cards listed all seem to be Police or Fire departments and I don't want to scan the card to the wrong place

Can anyone direct me to the correct place to add this card?

This is one of the Packer police sets from one of the many communities they released them from. I added this as a parallel set for you.
