Majkowski was born in Buffalo. The information is correct. I don't see where there's an error that needs noting. Just because not all of the possible information that could be listed isn't on there doesn't mean that it's wrong.
Marking it as a UER because not all of the information is there, or because it's displayed differently than other cards in the set, is going to create a nightmare for consistency. I can think of several examples of 1960s Reds cards that have different abbreviations of Cincinnati on the front, while others spell out the entire city. The information conveyed is the same, and correct - but displayed in a different way from card to card.
What about 2017 Topps baseball, where everyone has the city listed before the nickname, except for the Angels? Would that make each Angels card a UER?
Final example: 1990 Donruss (and I think 1991, too). These were a pain to pull examples of, but some have their full name on the back; some have just first and last, and I think some have middle initial (couldn't find any of these, but I'm not going to spend all morning looking through 1990 Donruss cards). Differences from one card to the next, when accurate information is displayed, do not constitute errors.
I realize that not all of my examples above are exactly the same as the original card in question here, but my ultimate point is that I believe UER notes (or ERR/COR, where appropriate) should be reserved for incorrect information.
